I think it's not the versioning issue because I found that the installer requests to uninstall the old version completely before installing the new version, thus I think it doesn't compare versions of individual DLL files when doing the upgrade.</div>

And in my case, even after I manually uninstall GnuCash, the stale DLL file is still there. This indicates that for some unknown reason the uninstaller might have lost track of some DLL files.</div>

After unstalling GnuCash and manually cleaning up, I can no longer reproduce the same issue from 5.12 - 5.14 - 5.16 upgrade cycle.</div>
