[patch] - Gettingstarted section for the Help doc

Attached is an update to the Help document.

Included is a "svn diff" for patching the current file, and a complete
"gettingstarted.xml" for replacing the current file in ...help/C for
those who want to view with yelp.

I moved the "accessing Help" to a separate heading at the front.

One question I have is that I added the e-mail addresses of the user and
developers.  Is this retrievable by spammer scanners?
