account tree and register window patch for stable branch

This patch combines my patches from today and from 2003-02-10.
I have it in the version of GnuCash that I use everyday and I
didn't notice any problems (yet). I think it's a worthwhile
improvement of the account tree, so I really would like to see
it in 1.8.2.
