Adjusting Balance

What to do if the balance sheet just does not balance?  I've checked and 
double checked.  I've checked in particular for transactions with missing or 
the same balancing account, for securities with missing information, and 
finally looked at all transactions entered after the last period in which the 
balances matched!

Does anyone have any ideas on how to track this down?  I have noticed a 
couple of things:

On the day the Balance is first incorrect I put in a security price that does 
not match the price at which I sold some of the same security that day.

The inbalance seems to be growing over time.
