crash on QFX import

This is a libofx bug

The fix is to patch libofx (lib/ofx_preproc.cpp)

I ran into this last night, but then I couldn't reproduce it this  
afternoon using the same ofx file, but a test gnucash file. So I'm  
not positive what's up, but the failure mode is the same as the bug  
description.
