Derek, I looked up the gnucash FAQ for csv2qif macro and clicked on the 
link given, http://gnucash.camfree.nl/rabocsv2qif, and arrived at a site 
that is not altogether aligned with GnuCash. You may like to check it out.

Where did you get csv2qif from?
