Depending on what you need, there are two flavors.

One flavor is when you customize a standard report with various options, change it's name in the "general" tab of the options dialog, and save it. That's pretty easy.

The other flavor involves writing a custom query (in Scheme), a custom options dialog (also in Scheme), and/or a custom format (you know by now what it has to be written in) and then save the whole mess somewhere that Gnucash can find it. There's some (woefully inadequate) documentation of this flavor in the reports sources, and a couple of examples as well under the "Sample and Custom" entry in the Reports menu.
