Duplicate customer name on tax invoice

I don't understand, where is it duplicated?  Perhaps you duplicated it
in your own datafile?  Did you enter in the Contact Name multiple times?
If you only entered it once then it will only print it once.  If you
entered it twice, then, there's your problem.
