Create sub-account to existing account with entries

If you look carefully in the edit account screen for any account, on the
left just above the account type is a check-box to select a property
called Placeholder.  When this is selected, the account cannot have any
transactions, only sub-accounts.  When not checked, transactions are
permitted.
