old and new Quote form

I had a crash and lost my invoice Form. I am trying to rebuild it but 
need help. Enclosed is the way I want it in Oldinvoice.pdf.
Pulliam1.pdf is what I am getting now. I don't know how to move the LOGO 
so it is above the Invoice Body.  Thanks for any help. Merry Christmas
