QIF import does not work

I've successfully imported QIF in previous versions of GC. The last one I did was in GC v2.6.0 built on 2013-12-29. My QIF file was formatted as follows, with some of the data at the end redacted with ellipses,

The QIF file was in Unix format. I'm on Windows 7 Professional 64-bit, Region and Language (Locale) format: English (United States) with Short date: M/d/yyyy.
I found an older QIF file in DOS format and just imported that one too. It asked for locale information and bank matching and took the data - unfortunately all of the transactions were already in my database, so I wound up with duplicates which I was prepared to deal with. This one was formated,

I am running MySQL for the back-end, but this should not matter as I've run XML in the past and never noticed any import/export issues related to the back-end (intuitively, they're independent).
