report to use as a work order

Derek,

The customer report only has the names and address of the customer.  I 
need the phone number, email, notes on the customer, etc. I also only 
want it for one customer.

E.g. When a customer comes in I have them fill out a sheet w/ name, 
addr, phone, email, login, and a description of the symptoms.  For 
existing customers and when I come in from on-site I would like to print 
their info. and if possible some text.

This would allow the customer to verify their info. and for me to use 
the sheet as a work order sheet to keep with their computer.

On another/similar note: where would I look for or create the scm/report 
directory.  At least one of the sample reports mention it but I don't 
find it when searcing ala

sudo find / -name scm
