Interesting Discovery in my accounts

Good Afternoon,
I just discovered that in 2014 a number of my expense accounts were not 
correctly closed (they had a balance other that ZERO)!

Questions that I have:

1. How did it happen?
2. Has this happened to anyone else?
3. How do I correct the problem?

I can not go back and recreate 3 YEARS of data.

Ideas...
