Graham, I do a couple of simple CSV imports each month.  These took some
trial and error to get right.

Please, some information from you first:
1.  Which version of GnuCash?
2.  Which O/S and it's version number?
3.  Itemize the columns in your csv file and verify how many columns.
4.  For each column in #3, identify the "Title" you picked from the
GnuCash list of column titles.

Now for some thoughts:
*  Any chance one of the columns before your amount field (credit?)
contains a comma (such as a description field) and isn't surrounded by
quotes?
* Have you tried to load this into Excel (or Calc for Libre Offce) to
see if any row looks strange?

I'll wait for your response.
