I don't know. How do the developers ensure that the shortcut is unique?
Trial and error, I'd say. This is exactly what the translators need to
do as well.

No, of course not. Unless people file bugzilla reports about that,
including the suggested alternatives. I've implemented suggestions from
there in the past, and I'd do so in the future as well.

But in principle this is IMHO a design flaw in the whole accelerator key
design -- or it rather needs fundamentally different tools to check
them. On some KDE news page there was talk about automated UI testing,
and checking the accelerator keys was one of the easy things an
automated UI tester could check easily...
