Patch update #2

Per Derek's suggestion that I send a weekly progress report:


1)      Using Christian's instruction about disclosing errors (xmllint -valid -noout gnucash-guide.xml), I found several errors.

2)      Am working on fixing my errors

3)      Also discovered that AppendixD of the guide has several errors.  The errors are in the original before I did anything.

4)      Errors reference the Data Type Definition (DTD) and I am trying to understand the specifics of that.  I may or may not be able to fix AppendixD errors.
