tax report questions

Hi.

I've been trying to generate a tax report for my wife's music-education 
business, and I ran into the following warning.  It consistently occurs only 
on the second generation of the tax report.  Also, if I subsequently try to 
export it to a TXF file, gnucash crashes.  I'm running 1.8.9 on gentoo.

Here's the warning at the top of my tax report:

WARNING: There are duplicate TXF codes assigned to some accounts. Only TXF 
codes with payer sources may be repeated.

Code "N315" has duplicates in 3 accounts:

Expenses:Equipment:Maintenance

Expenses:Equipment:Repair

Expenses:Equipment:Strings

Code "N301" has duplicates in 3 accounts:

Expenses:Gig Gear

Expenses:Miscellaneous

Expenses:Music

Code "N313" has duplicates in 3 accounts:

Expenses:Office Supplies

Expenses:Postage and Delivery

Expenses:Printing:Miscellaneous

Code "N302" has duplicates in 10 accounts:

Expenses:Bank Charge

Expenses:Books

Expenses:Childcare

Expenses:Dues & Subscriptions

Expenses:Education:Camp

Expenses:Education:Lessons

Expenses:Education:Recitals

Expenses:Education:Workshops

Expenses:Medical:Medicine

Code "N294" has duplicates in 2 accounts:

Expenses:Travel:Entertainment

I believe that it is complaining that I have set up multiple accounts to go 
the same tax form location.

1. What does it mean by "Only TXF codes with payer sources may be repeated."?  
Do i need to set up some payer information to make this work?  Is there a 
document that I missed somewhere that explains this?

2. Do I need to define my accounts so that they line up one-to-one with lines 
on tax forms, or is there a way for me to keep finer-grained accounts that 
get lumped together for the purposes of the tax report?

I really appreciate any help, especially before April 15  ;-)
