You already have all the accounts you need.
At this point, youy have a rebate from your Medical Expenses to your
Bank Account
Asset:Bank Account - $40 Deposit
Expense:Medical - $40 Rebate
That's all there is to it :) Adjust as necessary for your account
hierarchy complexity.
