Please remember to copy the list on all replies: Use "reply all" or, if your mailer supports it, "reply list". [Resent because I forgot to add the cc back in.]

Then you need to hire  an attorney to teach you how to do it. This is serious stuff and you can get in a lot of trouble if you screw it up. 

The spreadsheet I wrote is specific to the jurisdiction with which I must file and isn't likely to be of much help to you for yours, especially if you don't understand what your jurisdiction requires.
