Importing a csv file - Fails - Help required

on first load it came up with a list of missing info, tried to reload and  
then got an error saying date format or columns

So, I must be doing something wrong, have tried 10 rows, 1 row still the  
same.

Is there instructions on how to do a set-up from a csv file?
