I think you have 2 AccessAmerica accounts for some reason.  
Assets:CurrentAssets:AccessAmerica and Assets:AccessAmerica or something like 
that.

Have a look at the Expenses:Insurance:HomeOwners account and look carefully at 
the transfer account for the Argenia transaction on 1st Feb.
