You probably put the name in as both the customer and the contact.

Thank you Derek.  I did notice that I did not have the custom invoice
selected.

Second question- I am getting double names on the invoice- see attached.  Is
there a way around this?

Did you reload the report after you updated the style sheet?
Are you sure the report is using your new stylesheet?
