First thought is there is a 6 where there should be a 9 (or vice-versa).  
either with you or with the bank.  It may sound stupid, but It has happened to 
me.  I've spent ages trying to figure out a few pence and it was a transposed 
digit somewhere, that I just couldn't see for looking...
