Are you, by chance, using the “Import transactions from CSV” capability to update your accounts?

If so, you should be aware that sometimes the transaction matched is one that has already been reconciled, so it appears that the transaction has been added when, in fact, it has updated a previously reconciled transaction, replaced it as a “cleared” transaction, and then rereconciles it, leaving the account showing a missing transaction.
