I recently also met a similar issue that libwinpthread-1.dll is not updated after upgrading GnuCash. However I suggest a root cause different from your analysis: the uninstaller might have failed to remove the old file.
I think it's not the versioning issue because I found that the installer requests to uninstall the old version completely before installing the new version, thus I think it doesn't compare versions of individual DLL files when doing the upgrade.
And in my case, even after I manually uninstall GnuCash, the stale DLL file is still there. This indicates that for some unknown reason the uninstaller might have lost track of some DLL files.
After unstalling GnuCash and manually cleaning up, I can no longer reproduce the same issue from 5.12 - 5.14 - 5.16 upgrade cycle.
